But does the HGV medical exam test for drugs? The standard DVLA HGV medical (D4 form) doesn’t include a drug test. However, if you regularly use drugs or abuse substances (legal or illegal), this will be considered during the medical screening. As a result, your new employer will almost certainly ask for a separate test.

Is a Drug Test Part of the D4 HGV Medical Examination?
The D4 Medical is a physical exam, not a chemical screening. It’s a general health and fitness check to ensure you’re fit to drive and accounts for existing medical conditions. This can include:
- Eyesight testing
- Blood pressure checks
- Neurological exams
- General fitness assessments
The examiner won’t take blood or urine unless there’s a specific reason, such as something being flagged in your medical history.
However, if the examiner suspects drug misuse, and this goes against your declaration on the D4 form that you’re sober, they can ask for further tests before moving forward.
In other words, steer clear of anything that could affect your ability in an HGV driving job. You’ll be thankful for it later when you pass your medical and move ahead with your career.

HGV Medical Requirements: DVLA vs. Company Testing
It’s important to understand the difference between the DVLA medical and employer testing. Both the DVLA and your new employer want to make sure you’re drug and alcohol-free while you’re on the job. However, their roles differ slightly:
- The DVLA: The DVLA is concerned mainly with chronic, untreated drug or alcohol dependence that would compromise your ability to drive. Once you pass the DVLA medical, it’s no longer their concern.
- Employers: Employers have strict rules about drug and alcohol testing. This means that you will likely face random drug and alcohol tests (either urine or swab testing) throughout your time with the company, along with a test before you start your role.
The Non-Negotiable Reality of Safety and Licensing
As you’ll already know, driving is a safety-critical role. The risks increase significantly when driving a large vehicle like an HGV. While many people drink responsibly, excessive alcohol and illicit substances are strictly off-limits for drivers of large vehicles.
If you fail your DVLA medical due to alcohol or drug misuse, there’s a very good chance you’ll never be able to drive for a living. A “black mark” will be placed against your name. If you fail while already on the job, the consequences are even more serious.
Not only could you lose your job, but you could also face legal consequences such as the loss of your driving licence, heavy fines, or even prison time.
